As a trusted support coordination, we specialize in navigating Louisiana Medicaid Waiver programs, empowering individuals and families throughout Region 7, serving the parishes of Bienville, Bossier, Caddo, Claiborne, DeSoto, Natchitoches, Red River, Sabine, and Webster.

Support Coordination
The purpose of Support Coordination is to assist individuals and families in navigating OCDD and EPSDT Waivers Services, understanding their benefits, and accessing the necessary services for their well-being. Through assessments, referrals, and collaborative planning, our Support Coordinators work closely with participants and families to ensure they receive the care and support they need.

At Shepherd’s Heart our Support Coordinators develop and implement person centered known as CPOCS (comprehensive plans of care) tailored to each person’s specific needs. We partner with chosen service providers through Medicaid to create a safe and supportive environment by conducting regular provider check-ins, developing emergency plans, and ensuring all necessary services are in place.
