Shepherd's Heart is dedicated tp assisting individuals in gaining access to a wide range of opportunities and services that enhance their overall quality of life. From employment and volunteer opportunities to community involvement and transportation equipment we strive to provide comprehensive support. We also offer assistance with basic well-being, risk management for health, counseling services, relationship development, growth of interests and hobbies, self advocacy, and other essential areas to help individuals thrive.

ELIGIBILITY
SHEPHERDS HEART,LLC SEVICES ARE AVAILABLE TO:
- CHILDREN AND ADOLESCENCE AES 2-21 YEARS OLD
- INDIVIDUALS SEEKING SUPPORT NAVIGATING SERVICES FOR HEALTH DIAGNOSIS.
New Opportunities waiver:
Designed to assist those aged 3 and older who have developmental disabilities and wish to live independently in the community. (Individuals must be on the DD Request For Services Registry to participate. For more information, contact your regional OCDD Office).
children's choice wavier:
Offers additional service to children aged 18 or younger who have developmental disabilities and are either living at home or willing to return there. (Individuals must be on the DD Request For Services Registry to participate. For more information, contact your regional OCDD Office).
support waiver:
Provide job assistance to individuals who have developmental disabilities and services include day rehabilitation, pre-employment services, emergency relief, and supported employment. (Individuals must be on the DD Request For Services Registry to participate. For more information, contact your regional OCDD Office).
epsdt targeted population medicaid program:
Facilitates access to services for Medicaid eligible participants between the ages of 3 and 21 wo are on the DD Request for Service Registry to participate. ((Individuals must be on the DD Request For Services Registry to participate. For more information, contact your regional OCDD Office).
Communitychoice waiver:
Provides services to elderly and disabled individuals who qualify for for a nursing institution but would prefer to live in the community.
